On line 3, the value for the givens field is "Chirmin", the suggested correction is "Firmin". Submitted by Michel Dubois. The following comment was attached: It's Firmin as per BMS2000.
This correction was evaluated by Mark Gallop: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
