On line 50, the value for the surname field is "Rikely", the suggested correction is "Pikely".
This correction was evaluated by Willie Armstrong: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct. This comment was added: Hard to tell with the end of the first letter and start of the second, but Rikely is a fairly common surname in the area
