On line 4, the value for the givens field is "May", the suggested correction is "Mary". Submitted by Larry A Van Wormer. The following comment was attached: Not sure, but seems to match other "ry"s on the page.
This correction was evaluated by Mark Gallop: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct. This comment was added: May in 1906 and 1911
