On line 40, the value for the givens field is "Harvey", the suggested correction is "Havvey". Submitted by k.renneker. The following comment was attached: does not look like the 'rv' combination in Carver...looks like 2 v's because of the high connector
This correction was evaluated by Krystal Tait: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
