On line 31, the value for the givens field is "Elizabeth", the suggested correction is "Elizabeth D.(?)". Submitted by Shelley. The following comment was attached: there is an initial, but hard to make out clearly
This correction was evaluated by Geoffrey Wyght: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
