On line 12, the value for the givens field is "Na?????", the suggested correction is "Nazaire (?)".
This correction was evaluated by Jean-Marc Voisard: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct. This comment was added: You may be right but I'm still not convinced...
